Diabetes education

Diabetes is a health condition where there is too much glucose (sugar) in your blood. Symptoms can include feeling really tired, intense hunger or thirst, weight loss and urinating (weeing) more than usual.

When problems with your blood sugar are found early, you and your GP can take positive steps to prevent damage to your heart, kidneys, eyes, nerves, blood vessels and more. We offer a culturally safe diabetes education service for Aboriginal and Torres Strait Islander peoples in the Moreton Bay region. Our team walks alongside you to help manage your symptoms and to empower you to make deadly choices about your wellbeing and care.

What can we support you with?

Our diabetes educators can support you with: 

  • checking that your blood glucose meter is working properly 
  • driving requirements
  • healthy eating and physical activity 
  • helping you set targets for good health 
  • how your diabetes medications work
  • injection techniques and updates
  • keeping your diabetes in a safe place
  • what causes your blood sugar to be high, and what to do if it happens
  • what health check-ups you will need
  • what to do if your blood sugar is too low
  • what to do when you are sick

We can also work with other members of your healthcare team (such as your GP and specialists) on your behalf to ensure you receive the best care possible.

Our services

Our diabetes care clinic can provide all your yearly diabetes checks in one appointment.  

In a 90-minute appointment, you will see a diabetes educator, dietitian, podiatrist, optometrist, pharmacist and audiologist.  If needed, we can also link you in with a dentist, exercise physiologist, and social health worker to care for your mind and wellbeing.  

We will listen to your story and learn about your journey.  We acknowledge that you are the expert on your life and experiences. 

Where to start?

If you have had your 715 Health Check, or have a GP management plan, you can request a referral from your doctor to a diabetes educator.  

Or you can yarn with your doctor or nurse at our clinic about diabetes education.
